Complete the Basic Individual License Application.
Submit proof of photo identification (ID). More info
- Acceptable forms of ID include: a U.S.-issued driver’s license or non-driver’s license identification of Motor Vehicles; a license or permit issued by a government agency; a passport; an alien card/green card; a City, State, or Federal employee ID card; or a Municipal ID card.
If the applicant is not a resident of New York State, the applicant must submit certified proof of driving record. More info
- Acceptable proofs include:
-a Certified Abstract of the applicant's driving record issued within the last 30 days from the Department of Motor Vehicles of the state, territory, district, country (or political subdivision) that issued the license that demonstrates the license is valid and in full force and effect OR
-other certified proof issued within the last 30 days from the territory, district, or country (or political subdivision) that issued the applicant's driver's license that demonstrates that the license is valid and in full force and effect
If the license application will be filed by someone other than the applicant, complete the Granting Authority to Act Affirmation.

- $35: License Fee for applicants who file for a new pedicab driver license from May 1 to November 1.
- $18: License Fee for applicants who file for a new pedicab driver license from November 2 to April 30.
- Non-Refundable Convenience Fee for paying with a credit card.

After submitting an application online, print a copy of the Confirmation Page. The Confirmation Page will list the additional steps and materials required to complete the application. These materials are also listed above and must be submitted within five business days of filing the application online if they were not uploaded as part of the online application submission.
